Background: Whether to operate on asymptomatic patients with congenital lung malformations (CLMs) remains controversial. Our study intended to find out the proportion of hidden infection in CLMs and its effect on surgery, to provide help for the management of asymptomatic CLMs patients. Methods: A retrospective review of the medical records of patients with asymptomatic CLMs from January 2011 to December 2020 was performed in our center. Selected asymptomatic patients were divided into a non-hidden infection group (NHI) and a hidden infection group (HI). Results: A total of 581 asymptomatic CLMs patients were included in this study. Thirty-two percent of asymptomatic CLMs patients had hidden infection in the lesion. Among various CLMs diseases, intralobular pulmonary sequestration had the highest percentage of hidden infection (48.8%). With age, the proportion of HI gradually increased. Patients in the HI and NHI groups were 223 and 121. The incidence of pleural adhesion and focal abscess in the HI group were 14.9 and 7.4%. Statistical significances were shown between the two groups in intraoperative blood loss (p = 0.002), operation time (p = 0.045), chest tube drainage time (p < 0.001), postoperative hospital stay (p < 0.001), and air leak (p = 0.012). Conclusion: The proportion of HI detected by postoperative pathological results was high and they could increase the difficulty and risk of surgery. Therefore, early surgery may be a more appropriate choice for the management of asymptomatic CLMs patients.

Resumen El virus de la hepatitis B (VHB) tiene un gran impacto mundial. No obstante la disponibilidad de la vacuna, 2000 millones de personas se han infectado agudamente y, de ellos, 240 millones persisten crónicamente infectados. La infección tiene diferentes formas de presentación tales como infección aguda, infección crónica, infección oculta y reactivación cuando hay inmunosupresión. Así mismo, hay marcadores muy sensibles como el anticore, cuya positividad puede tener diversos significados. El recientemente descrito antígeno relacionado con el antígeno core es un marcador emergente que podría reemplazar al ácido desoxirribonucleico (ADN) viral. En la presente revisión se discuten los exámenes de laboratorio necesarios para el diagnóstico de los diferentes escenarios de la infección.


Abstract Hepatitis B virus (HBV) has an enormous global impact. Despite the availability of a vaccine, two billion people have been acutely infected. Of these, 240 million remain chronically infected. The infection has different forms of presentation including acute infections, chronic infections, hidden infections, and reactivation when there is immunosuppression. Similarly, there are very sensitive markers such as anti-core, but a positive test can have different meanings. This recently described antigen which is related to the core antigen is an emerging marker that could replace viral DNA. In this review we discuss the laboratory tests necessary for diagnosing the various scenarios of the infection.

Introducción: Los pacientes en tratamiento con hemodiálisis constituyen un grupo de alto riesgo para la infección por virus de hepatitis B y C. Objetivo: Identificar marcadores serológicos y moleculares de infección viral de hepatitis B y C en pacientes hemodializados. Material y métodos: Se realizó un estudio observacional, descriptivo, transversal que incluyó a 103 pacientes con IRC sometidos a régimen de hemodiálisis de las regiones occidental y central de Cuba. Las muestras fueron recibidas entre enero y abril del 2016 para estudio de marcadores serológicos y moleculares de hepatitis B y C a realizar en el Instituto de Gastroenterología. Se estimó la seroprevalencia de anti HCV, HBsAg y Anti S, además se realizaron determinaciones de carga viral mediante prueba de amplificación de ácidos nucleicos para la cuantificación de ADN y ARN para virus de hepatitis B y C respectivamente. Las asociaciones fueron evaluadas mediante el estadígrafo x2. Resultados: EL 7.8 por ciento de los hemodializados fueron portadores inactivos de hepatitis B. El 70.8 por ciento de los pacientes poseían marcadores de infección por virus de hepatitis C con viremia oculta en 18.4 por ciento de los mismos. Conclusiones: Elevada prevalencia de infección y viremia oculta por virus de hepatitis C en los pacientes hemodializados(AU)


Introduction: Patients subjected to hemodialysis treatment are a high risk group for hepatitis B and C infection.Objective: To identify molecular and serologic markers of hepatitis B and C viral infection in hemodialysis patients. Materials and Methods: Was performed an observational, descriptive, cross-sectional study including 103 patients with chronic kidney disease undergoing hemodialysis regime from Cubas Western and Central regions. The samples were received between January and April 2016 to study serologic and molecular markers of hepatitis B and C to be performed at the Gastroenterologys Institute. Seroprevalence of anti HCV, HBsAg and Anti S was estimated, plus viral load determinations using amplification test to measuring nucleic acids DNA and RNA for hepatitis B and C respectively. Associations were evaluated using the x2 statistician.Results: 7.8 percent of hemodialysis patients were inactive carriers of hepatitis B. The 70.8 percent of patients had infection markers of hepatitis C virus; being hepatitis C hidden viremia in 18.4 percent of them. Conclusions: High prevalence of infection and hepatitis C hidden viremia in hemodialysis patients(AU)
